When in a special alternate universe where there is no Constellation, the buffs and debuffs from having Sustenance turned on in the Gameplay Options menu, are not applied to the character.

Reproduction Steps

Turn on Sustenance in the Gameplay Options menu. Go through the Unity and end up in a universe where there is no Constellation. Notice that "Wellness Added" does not pop up in the corner of the screen, and in character status, the Fed and Hydrated buffs are not there.
